Where we start

BCBS commercial plans first.

We take on your BCBS commercial claims first. Other payers and secondary claims follow, one payer at a time, once their rules pass our checks.

We check the patient’s coverage before we file. A coverage problem shows up before the claim goes out, not weeks later on a denial.

Coding review

Each CMT claim checked against the signed note.

Codes 98940 to 98942. We read the note. We never change it.

  • The AT modifier. On each CMT line the note supports as active treatment.
  • The regions match the code. 98940, 98941 or 98942 matches the number of spinal regions in the signed note.
  • The M99 segmental dysfunction code first. One for each region adjusted, listed before anything else.
  • Symptom codes second. Pain and other symptom codes follow the M99 codes.
  • The initial treatment date on the note. We read it from the signed note, where the payer expects it.

When the note does not support the claim, the question goes to your provider, once. Your provider decides what the note says.

Then the rest

Filed, followed, posted, matched.

  1. Filing. Each claim goes out through the clearinghouse.
  2. Follow-up. We track every filing deadline and every claim that goes quiet.
  3. Posting. We post each payment into your EHR and keep every adjustment.
  4. Patient balance. We work out what the patient owes from the payer’s payment and explanation.
  5. Deposit match. We match each bank deposit to the payment behind it.

What you get back: paid claims, and a short list of what needs you.

One launch fee, quoted with your scope, covers enrollment, connecting your EHR, capturing your contract rates and the shadow period.
